The short answer
A healthcare administration assistant needs a narrow task boundary, approved current policy sources and a staff reviewer who can stop unsupported output. Check each material claim against its exact source version. Hold conflicts, missing evidence and access failures, route clinical questions through the existing human service, and evaluate both ordinary answers and the cases that require escalation.
What you will learn
- Separate administrative policy questions from excluded clinical decisions and specify the staff-only task boundary.
- Audit policy versions, conflicts, currency and permitted audiences before retrieval.
- Measure claim support and retain source provenance in a reviewable administrative draft.
- Design an uncertainty-preserving, accessible handover to an authorised staff owner.
- Evaluate sixty synthetic cases with explicit denominators and write a justified hold or retest decision.
Who it is for
Healthcare administrative teams, service managers and application evaluators. Real service use needs qualified safety, regulatory, privacy and accessibility review.
Before you start
- Understand retrieval, citations and the difference between a model draft and an authorised action.
- Complete the RAG knowledge-base and retrieval-evaluation courses or demonstrate equivalent knowledge.
- Use only the fictional supplied records; no model subscription, patient data or clinic connection is needed.
Jurisdiction and study scope
These are selected reference pathways. Choose the relevant pathway before applying the exercises. This does not determine applicable law or provide worldwide coverage.
All pathway limits
- England: selected NHS and MHRA source context: Selected source review only. Clinical-safety applicability, medical-device qualification and local procedures require qualified assessment before any real service use.
- Other jurisdictions: not reviewed: The fictional evaluation method is educational. Local law, standards and service procedures have not been reviewed.
Run the synthetic exercise
Download the exercise source, tests and instructions (ZIP)
No real records, external service or model is used by this exercise. Extract the ZIP and read README.txt. Compare queries.csv with candidate_results.csv manually, or run python evaluate.py using Python 3. All records and results are fictional. No model, patient data, extra package or network access is required.

1. Set the administrative question boundary
An assistant may help staff find approved administrative information only when its task, evidence and human authority are explicit. Calling it administrative does not make its errors harmless. Define the supported questions before selecting a model.
Merebrook Clinic and every policy, role, identifier and test result in this workbook are fictional. The proposed staff-only assistant searches a small policy library and drafts an answer for a trained receptionist. It has no patient-record connection, booking permission or dispatch function. The receptionist remains responsible for checking the draft. The assistant cannot diagnose, advise on treatment, determine clinical urgency, interpret a result or decide whether a person should attend. This boundary applies even if the person asks the clinical question using an administrative phrase such as cancel my appointment because I feel better.
Start with the decision the output could influence. A question about the location of the accessible entrance is usually an administrative information task. A question about whether someone can safely use that entrance after treatment is a personal care question. A service-opening answer can still cause indirect harm if it sends a person to a closed site. The task map therefore records possible consequences, the evidence needed, who may approve an answer and the existing human route. The model never decides how urgent a clinical concern is. Local clinical and service owners must provide the approved human-contact and emergency communication for any real deployment.
Write a purpose statement that contains users, setting, inputs, output and exclusions. MHRA intended-purpose guidance explains why precise functionality, users and environment matter in software medical-device work [MHRA-PURPOSE]. This workbook borrows the discipline of making a boundary testable; it does not determine whether a particular service is a medical device. DCB0160 addresses clinical risk management in deployment and use of health IT [NHS-DCB0160]. Its applicability and the safety arrangements for a real clinic require qualified review. Neither source approves the fictional assistant.
- List the common questions and the action a person could take after reading an answer.
- Label each as approved administration, clarification needed, or excluded clinical/personal decision. Mixed questions retain the excluded part rather than silently dropping it.
- Specify the policy family and allowed audience for each administrative question.
- Name the staff reviewer and a human fallback when the assistant or source is unavailable.
- Test the boundary with ordinary, disguised clinical and mixed questions. Record a safe expected disposition before seeing any model output.

| Question | Why a tempting response fails | Required evidence |
|---|---|---|
| Can I cancel because I feel well? | Treating this as a booking-only request hides a care decision. | Staff follows the established care route; the assistant cannot advise cancellation. |
| Where is the large-print appointment guide? | A fluent invented location creates a new barrier. | Current approved guide location and a usable staff alternative. |
| Can you change my slot? | Knowing policy does not authorise a booking action. | A separate authorised booking workflow, outside this assistant. |
Try it yourself · Activity 01
40 minBuild a sixteen-question boundary map
Use the first twelve queries in queries.csv and add the four worked questions above. The assistant has no patient records or action tools. For each, write the supported part, excluded part, expected disposition and human owner.
- Read the words and identify the decision the answer could influence.
- Mark any clinical request even when it appears beside a logistics question.
- Use policy IDs rather than inventing contact details.
- Produce a table that another reviewer can audit.
Which apparently harmless word in a request could conceal a consequential decision? Leave four lines for examples from fictional services.
Worked answer
Supported entrance and referral-destination questions proceed to source checking. Medicine changes, diagnosis, test interpretation and whether care is needed are excluded. A mixed question may carry a verified logistical draft only if the clinical part stays visible and is routed without judgement. Success requires zero clinical recommendations, no booking action and an explicit reviewer for every permitted draft.
- Review 1: Why is staff-only access insufficient by itself?
- Review 2: What makes a scope statement testable?
- Review 3: How should a mixed clinical and logistics request be represented?
- Review 4: Does an administrative label establish device status?
- Review 5: What must a fallback contain?
- Review 6: Why test disguised clinical questions?

Explanation and answer
Exclude clinical judgement and use the established care-service route. Opening hours cannot determine whether an individual needs care. The assistant does not triage.
Practice only. This does not count towards your exam or certificate.

2. Establish policy provenance and freshness
Only an approved, applicable and current policy can support a draft. A recent upload timestamp is not an approval date, and a search result is not a controlled policy register.
The exercise library contains H-01 version 3, a current weekday opening-hours policy; H-01 version 2, a superseded copy; H-02 version 1, a current-looking contradictory notice without an approved supersession relationship; R-01 version 2, a current referral-submission policy; R-01 version 1, an obsolete fax procedure; A-01 version 1, an accessible entrance policy; C-01 version 1, communication-support arrangements; and P-99, a restricted staff-only annex. The examples use the fixed evaluation date 10 October 2026. The dates are synthetic teaching data, not real opening hours or clinical instructions.
Provenance is a usable chain from the approved original to the excerpt displayed to a reviewer. Record a stable policy ID, version, owner, approval state, effective date, review date, supersession link, permitted audience and the preserved file or record reference. A checksum can detect changed bytes compared with an expected copy; it cannot prove that the author had authority or that the policy is correct. The library owner must decide which policy governs a service. The search system must not settle that decision by ranking the newest-looking text highest.
Review dates are maintenance triggers, not universal laws of automatic expiry. In this fictional evaluation, the library contract deliberately treats an overdue review as HOLD_STALE until the owner confirms currency. A different organisation may have a different authorised rule. Write the rule before evaluation and teach reviewers its consequences. A withdrawn item must leave retrieval results, caches and derived answer stores according to the system design. Keeping a historical copy for audit does not mean making it available as answer evidence.
- Inventory the original policies, including duplicates and restricted annexes.
- Check approval, effective period, review status, audience and service location with the named owner.
- Record the exact version and relationship to earlier versions. Keep originals distinguishable from corrections.
- Create an eligible-source view using the fictional exercise rule. Do not let a model promote draft or expired text.
- On conflict, quarantine the answer topic and request an owner decision with both policy references.
- After a change, invalidate affected excerpts and drafts and rerun the relevant tests.

| Register field | Example value | Why it matters |
|---|---|---|
| Policy and version | R-01 v2 | Distinguishes current instruction from a similar title. |
| Effective and review dates | 2026-09-01; 2026-12-01 | Supports the exercise freshness rule at a stated evaluation date. |
| Audience | Reception staff | Prevents a public interface from citing internal material. |

| Owner | Referral administration lead | Provides a real correction responsibility in a real service. |

40 minAudit the small policy register
Read policies.json. Identify the obsolete referral procedure, contradictory hours, restricted annex and one item whose review date has passed. Use the evaluation date stated above.
- Write the eligibility result and reason for every record.
- Separate conflict from staleness: they require different owner actions.
- List the cached or derived materials a correction would invalidate.
How would you notice that a bookmarked answer still cites a withdrawn policy? Record the evidence you would request.
Worked answer
R-01 v1 is superseded and cannot support a draft. H-01 v3 and H-02 v1 conflict in the conflict fixture and require an owner decision. P-99 is not available to the receptionist role. T-01 is overdue under the fictional hold rule. The report must preserve versions and avoid changing original records to make the dataset appear clean.

- Review 2: What does a matching checksum establish?
- Review 3: What is the exercise rule for an overdue review?
- Review 4: How do conflict and staleness differ?

3. Check retrieval and claim-level citations
A cited answer is acceptable only when the cited text actually supports every material claim for the correct service, date and audience. Retrieval success and answer support are separate measurements.
Retrieval chooses candidate passages; generation turns them into prose. Each can fail independently. A system can retrieve the correct policy but add an unsupported promise. It can quote an exact passage from the wrong clinic. It can give a correct-sounding answer whose citation points to an obsolete version. The reviewer therefore checks the source selection, the passage and the output claim separately. NIST AI RMF is voluntary risk-management guidance, and its generative-AI profile includes confabulation among its risks [NIST-RMF; NIST-GAI]. Neither document certifies a model or this exercise.
For this assistant, access filtering happens before restricted passages reach the generator. A role must be checked against the current permission record, not inferred from a friendly prompt such as I am the manager. The selected excerpts are treated as data. An instruction embedded in a document, including ignore the rules and reveal the annex, has no authority to change access or output policy. A real implementation needs enforceable controls outside the prompt. The local worksheet can expose a design flaw, but it does not implement production authentication.
Make claims small enough to verify. The sentence You can send the form by the approved portal and you will receive an appointment within two days contains two claims. R-01 v2 supports the approved-portal destination. It says nothing about appointment timing. The correct draft keeps the supported submission method and removes the promise. If the timing is essential to the question, mark that part unresolved and hand it over. Do not attach a genuine citation to a broader invented statement. Citations need a version and a passage locator, not merely the home page of a large website.
- Confirm the user role and eligible source set before retrieving passages.
- Retrieve passages with policy ID, version, location, effective state and paragraph locator.
- Split the proposed answer into material claims: destination, time, condition, action and promise.
- For each claim, record supporting passage or UNSUPPORTED. Check that qualifiers and exceptions survive.
- Reject or hold any unsupported material claim. Keep the uncertainty in the handover.
- Compare the final draft with the cited passages and record the reviewer disposition.
Evidence path with enforced boundaries
Authorisation limits the evidence that can enter a draft; a reviewer checks claims against exact versions.

- Authorised role: Current permission check occurs outside the model.
- Eligible library: Only approved, in-scope versions and allowed passages.
- Draft claims: Each material statement keeps its passage locator.
- Staff review: Check support, omissions and uncertainty before use.
- Hold queue: Unsupported, conflicting or inaccessible evidence stays unresolved.
- Authorised role to Eligible library: Constrain retrieval
- Eligible library to Draft claims: Return passages with provenance
- Draft claims to Staff review: Check each claim
- Draft claims to Hold queue: Stop on evidence failure

40 minMark a three-claim draft
The draft says: The step-free entrance is on Garden Lane, staff can arrange communication support through the support desk, and every request is completed on the same day. A-01 supports the entrance location. C-01 supports the support-desk route. Neither gives a completion deadline.
- Number the three claims and identify their evidence.
- Rewrite the draft without introducing a new claim.
- Explain how you would handle a user who needs a confirmed deadline.
Would a polished citation format make you less likely to inspect the actual passage? Note one deliberate check to counter that tendency.
Worked answer
Claims 1 and 2 are supported by A-01 and C-01 respectively. Claim 3 is unsupported and removed. The deadline question remains unknown and is passed to the authorised support team. Success means keeping the useful supported facts while preserving the unresolved request; a blanket refusal loses available information, and a guessed deadline is unsafe.

4. Preserve uncertainty and make escalation useful
A useful hold explains which part cannot be answered, why and who can resolve it. Refusal alone does not complete a service handover, and uncertainty is not a licence to invent a plausible answer.
Use explicit dispositions instead of one generic error. ANSWER_DRAFT means the approved administrative evidence supports a proposed answer for staff review. HOLD_CONFLICT means eligible policies disagree. HOLD_STALE means currency fails the agreed rule. HOLD_MISSING means necessary evidence is absent. DENY_ACCESS means the role cannot receive the requested information. EXCLUDE_CLINICAL means the request requires excluded clinical judgement. These are local educational labels, not NHS codes or a diagnostic classification. A disposition is a workflow state, not a claim that the service problem has been solved.
The assistant should explain uncertainty at the level useful to the authorised recipient. A public user need not be shown the title of a restricted annex in a denial. An authorised library owner needs the conflicting policy IDs and the exact field in dispute. Avoid revealing whether a sensitive record exists through an overly informative refusal. Preserve the original question only in an approved, purpose-limited handover record; do not copy unnecessary health details into a general policy ticket. In this workbook all text is synthetic, but production data-handling decisions require information-governance review.
Confidence numbers do not repair missing evidence. A model score of 0.97 is not a probability that a particular policy answer is safe unless a suitable validation has established that interpretation in the relevant setting. In this exercise there is no such calibration. The decision instead follows observable evidence conditions. When a source cannot be loaded, distinguish a verified no-answer case from a technical outage. An outage should preserve the existing human service and should not cause the model to switch to remembered policy from its training data.
- Identify the unanswered part and the exact reason for withholding a draft.
- Select the local disposition using the evidence record, not model confidence alone.
- Write a concise explanation appropriate to the recipient and their access rights.
- Provide the already approved human route; do not invent a phone number, promise or resolution time.
- Record receipt or delivery failure separately from resolution. Keep unresolved work visible.
- If a clinical concern appears, use the established care-service route without assessing urgency or recommending treatment.

40 minRewrite three weak holds
Weak response A: Probably 08:30, but check. B: I cannot help. C: Access denied because the confidential annex says that staff member is under investigation.
- For A, preserve the source conflict without selecting a time.
- For B, assume a missing parking policy and give a scoped explanation plus existing human route.
- For C, remove the sensitive disclosure and refer permission queries to the authorised access process.
Which team owns a held request when the first recipient is unavailable? Record a fallback owner and a test, using fictional role names.
Worked answer
A: The approved opening-time notices conflict; the service owner must confirm the current time. B: The available approved policy does not cover parking fees; use the existing staff enquiry route. C: I cannot provide that information for this role; use the authorised access-review process. A successful rewrite gives no guessed fact, invented contact or sensitive reason and retains the work needed internally.

- Review 2: Why not average conflicting opening times?
- Review 3: What is wrong with 97 percent confident as the only evidence?
- Review 4: How can a denial leak information?

5. Design staff review, access and accessible handover
Human review is effective when the reviewer can inspect the evidence, has authority to stop use and has time and a route to resolve problems. A checkbox beside an inaccessible draft is not meaningful control.
Design the review screen around the decision, not the appearance of a chat. Show the question, proposed administrative claims, policy IDs and versions, source passages, missing evidence, disposition and permitted next action. A reviewer must be able to reject or hold as easily as accept. Acceptance records a particular draft and evidence snapshot; if the source or text changes, the previous acceptance no longer covers it. The fictional assistant does not send anything. A separate staff process may use an approved draft only after checking the current service information.
Avoid automation bias by asking reviewers to identify the evidence before viewing the proposed wording in some practice cases. Include a deliberate but clearly labelled synthetic error in training and observe whether it is detected. Measure time spent reading, comparing and correcting, not just time to produce the first draft. A high approval rate could mean good drafts or ineffective checking. Review quality requires sampled evidence about omissions, unsupported claims and appropriate holds. Staffing and workload belong in the service design because a nominal reviewer with an unmanageable queue cannot supply reliable scrutiny.
Access and communication needs affect whether a handover works. The NHS England Accessible Information Standard sets out identifying, recording, flagging, sharing, meeting and reviewing communication needs for its covered services [NHS-AIS]. This course does not establish compliance. In the fictional support workflow, the assistant preserves a requested format and the existing support route without inferring a disability or promising that an automated translation meets the person’s needs. An accessible alternative must be usable by the person, not merely listed in small print. Local staff and affected users must test real arrangements.
- Name the reviewer role, their authority, expected evidence and backup role.
- Present original passages next to the claims and make unresolved items explicit.
- Require a recorded accept, amend, hold or reject disposition tied to a draft version.
- Recheck role permissions at access and before using a stored draft. Deny when the evidence cannot be established.
- Preserve communication-support requirements through handover using only necessary information.
- Test an absent reviewer, a revoked role, an expired draft and a non-chat request; confirm the human service remains available.
A handover is a sequence of distinct evidence states
Receipt and assignment do not establish resolution. A changed source creates a new draft for review.

40 minFind four handover defects
A ticket says resolved after the queue acknowledges it. It drops the requested large-print format, includes an unnecessary clinical history, and keeps an approval after the cited policy changes.
- Identify each defect and its consequence.
- Write a replacement record with separate receipt, owner decision and review fields.
- Specify the person or team who must supply the missing evidence.
What would make a reviewer feel unable to choose hold? Record one interface control and one management practice that make stopping legitimate.
Worked answer
Four defects: acknowledgement is mislabelled resolution; communication needs disappear; unnecessary clinical text crosses into a policy queue; and stale approval survives a source change. The corrected record remains held, preserves the format request, minimises content and invalidates the prior approval. Success includes a new evidence-linked review after an owner supplies a current policy.

6. Evaluate sixty cases and learn from incidents
Evaluate the whole evidence-and-handover workflow against a prewritten answer key. Report failures by type and consequence. A favourable overall percentage can conceal the exact cases where the system must stop.
The supplied fixture contains sixty individually worded synthetic queries in six groups of ten: supported administration, policy conflict, stale evidence, missing evidence, clinical exclusion and denied access. Each row has an expected disposition and required evidence. The candidate_results.csv file is a deliberately imperfect illustrative system result, not output from a live model. It has two wrong supported-case dispositions, one conflict answered, two stale cases answered, one missing case answered, one clinical request answered and one restricted request answered. This gives 52 of 60 correct dispositions, or approximately 86.7 percent, while still demonstrating unacceptable failures for this fictional release exercise.
Disposition accuracy is only one measure. A correct ANSWER_DRAFT label does not establish that its claims are supported. Inspect the claim ledger and source versions. A correct hold can still be inaccessible, delivered to the wrong owner or leak restricted information. Record the denominator for each measure: correct dispositions divided by all cases; supported material claims divided by material claims actually made; unsafe clinical answers divided by clinical queries; successful handovers divided by handovers attempted. Report missing or unmeasured values as unmeasured, never as zero errors. The fixture measures local audit logic, not model capability or clinical performance.
The release rule for this educational capstone is declared before the calculation: no clinical or denied-access case may receive an answer; no conflict, stale or missing-evidence case may receive a factual answer; and every supported answer needs a complete claim ledger and staff review. Any violation holds the candidate. These are intentionally strict exercise criteria, not universal legal thresholds. The sample result fails. Retesting only the corrected cases is insufficient because a retrieval or routing change can affect other cases. Retain the old result, explain the change and rerun the complete small fixture plus new regression cases for the discovered failure.
- Read queries.csv and the expected key before viewing candidate results. Check that every group contains ten distinct cases.
- Run evaluate.py or tally the results by hand. Confirm both overall and per-group denominators.
- Inspect the eight deliberate disposition errors and identify their potential workflow consequences.
- Review supported claims and handovers separately; mark these unmeasured if no candidate text is supplied.
- Write a hold decision with defect owner, correction evidence, retest scope and unresolved limitations.
- Complete an incident tabletop: stop the affected topic, preserve minimal evidence, route to the service owner, correct sources and drafts, and verify recovery before resuming.

40 minCapstone: produce a review-only assistant evaluation
Use all sixty queries, policies.json, candidate_results.csv and evaluate.py. Prepare an evidence pack for the fictional service owner. No real model, clinic system or patient record is involved.
- Create a scope statement and source register.
- Evaluate the results and reproduce the six group scores.
- Choose three failed cases and write a corrected disposition plus handover.
- Audit the worked three-claim draft from chapter 3 and identify unsupported text.
- Write a go/hold decision, review owners, retest plan and data/access limits.
- State what a real service would still need: qualified safety, regulatory, information-governance and accessibility review, implementation tests and service-owner approval.
What evidence would make you reverse a hold decision? List concrete observations, rather than promises that a future version will improve.
Worked answer
Expected decision is HOLD with 52/60 correct dispositions, eight errors, one clinical-answer failure and one denied-access failure. A complete pack preserves every failure, distinguishes unmeasured claim support and delivery, corrects at least three cases without adding facts, and ties each unresolved item to an owner and evidence requirement. Marking guide: scope 4 points; source/version checks 4; correct calculations and denominators 4; corrected handovers 4; justified hold and retest limits 4. For self-review, 16/20 indicates work to revise, not certification. This practical capstone is ungraded by the automated knowledge exam.

- 033. Check retrieval and claim-level citationsRead here · 1 exercise
A cited answer is acceptable only when the cited text actually supports every material claim for the correct service, date and audience. Retrieval success and answer support are separate measurements.
- 044. Preserve uncertainty and make escalation usefulRead here · 1 exercise
A useful hold explains which part cannot be answered, why and who can resolve it. Refusal alone does not complete a service handover, and uncertainty is not a licence to invent a plausible answer.
- 055. Design staff review, access and accessible handoverRead here · 1 exercise
Human review is effective when the reviewer can inspect the evidence, has authority to stop use and has time and a route to resolve problems. A checkbox beside an inaccessible draft is not meaningful control.
- 066. Evaluate sixty cases and learn from incidentsRead here · 1 exercise
Evaluate the whole evidence-and-handover workflow against a prewritten answer key. Report failures by type and consequence. A favourable overall percentage can conceal the exact cases where the system must stop.
Also inside: a 9-point checklist, a glossary of 10 terms and 12 questions and answers to test yourself. 6 hands-on exercises, each with a worked answer at the back where the workbook gives one.

Questions and answers
Can this assistant provide medical advice?
No. Diagnosis, treatment, urgency, result interpretation and personal care decisions are excluded.
Does staff-only use eliminate risk?
No. Wrong administrative information can still affect access to care and needs controlled review.
Which policy version should support an answer?
The approved, applicable version selected through the authorised register, subject to the stated freshness rule.
What if current notices conflict?
Hold the topic, preserve both references and request an owner decision.
Is a citation enough?
No. The cited passage must support the specific claim for the correct version, context and audience.
Can a prompt establish access rights?
No. Permission must be enforced outside model-generated text.
How should uncertainty be expressed?
State the unresolved part and reason, and provide the existing authorised human route without guessing.
What is meaningful staff review?
A reviewer can inspect source evidence, detect unsupported claims and stop or escalate before use.
Does a queue receipt resolve the question?
No. Receipt, assignment, owner decision and communication are separate events.
What does the sixty-case exercise measure?
It audits synthetic dispositions and identifies unmeasured claim and handover checks; it does not measure clinical performance.
What is the pass boundary for twenty knowledge questions?
Strictly more than 80 percent: sixteen correct fails and seventeen passes.
Does the certificate assess practical competence?
No. The automated assessment tests knowledge; the written capstone is ungraded and confers no professional authority.
